Basic Procedures

Breast augmentation is the most popular cosmetic surgery in the U.S. Each year about 300,000 women undergo this procedure. Breast augmentation surgery improves the size, shape, and firmness of the breasts through the placement of implants filled with silicone gel or saline (saltwater solution).

Incision Placements

Periareolar: Along the lower border of the areola.
Infra-mammary : Underneath the breast.
Transaxillary: Through the armpit.
Transumbilical: Through the belly button.

Breast implants have traditionally been placed through a number of incisions, even through the belly button. We usually use a crease or IMF (infra-mammary fold) incision as it is believed to be associated with the lowest link of revision surgery from capsular contracture or implant malposition.

We generally place the implants under the muscle layer because of the following reasons:

Types of Implants

Saline implants

Saline implants are made with a silicone outer shell that is filled with sterile saltwater after placement. They are approved by the FDA for individuals aged 18 and older.

Silicone Gel Implants * Most Commonly Used *

Silicone gel implants also use a silicone shell, but they are pre-filled with a soft silicone gel designed to closely mimic natural breast tissue. These implants are FDA-approved for patients 22 and older.

Silcone Gummy Bear or Form Stable

Highly cohesive silicone implants, often referred to as “gummy bear” or form-stable implants, contain a firmer silicone gel that holds its shape even if the shell is compromised. Like traditional silicone implants, they are approved for patients 22 and up.

Fat Grafting

Fat grafting, also known as fat transfer, is when fat cells are harvested from another part of the body and then ‘transferred’ to the breast. This has the associated risks and benefits of liposuction, and the size changes achievable are typically very modest.

I had my breast explant surgery done at the Plastic Surgery Institute of New Mexico with Dr. Knotts, and I truly could not be happier with my experience. From the very beginning, Dr. Knotts took the time to carefully measure everything to ensure my breasts would be as even and symmetrical as possible. Performing lymphatic self massage following videos and hiring a masseuse who specializes in post surgery scar tissue management and lymphatic massage was worth every penny. The surgery staff was also very supportive throughout the process with my questions and documentation for work.

It did take a few months for the swelling to go down, but the final outcome was absolutely worth it. Patience required.
